The Day-Use Rental Agreement and Risk Acknowledgment records the terms of a private half-day, full-day, or other approved daytime reservation at West Hollow Cabin and The Meadow.
The Agreement explains the off-grid nature of West Hollow, identifies the areas and activities included in the reservation, and describes the natural and activity-related risks a guest should understand before visiting. It also incorporates the West Hollow & The Meadow Guest Rules & Property Instructions.
Completing this form does not create a reservation or give the guest permission to use optional areas, fires, equipment, or activities. A reservation is accepted only after Oak Hollow Cabins has confirmed the terms, received any required payment, reviewed the completed Agreement, and countersigned it.

Download and Complete the Agreement
First, contact Oak Hollow Cabins and receive confirmation of the proposed reservation date, time, price, and approved activities.
Then tap or click the DOWNLOAD button below the document. The form may not be fillable while displayed inside the website preview.
After downloading the PDF:
- Open it in Adobe Acrobat Reader or another PDF application that supports fillable forms.
- Enter the Agreement date and the version date shown on the current Guest Rules & Property Instructions.
- Enter your full legal name, phone number, email address, and emergency-contact information.
- Complete the reservation date, arrival time, departure time, payment information, and other reservation fields exactly as confirmed by Oak Hollow Cabins.
- Do not select the indoor wood stove, outdoor fire ring, The Clearing, gorge access, a pet, alcohol, equipment use, or another optional activity unless Oak Hollow Cabins has specifically approved it.
- Read all five pages of the Agreement and the complete Guest Rules & Property Instructions.
- Initial the risk-acknowledgment section on page 3.
- Check each acknowledgment box on page 4 and enter your initials.
- Enter your printed name, electronic signature, and date on page 5.
- Leave the Owner/Host signature and date fields blank unless Richard instructs you otherwise.
- Save the completed PDF to your device.
- Reopen the saved PDF and confirm that all entries, checkmarks, selections, initials, and signatures were retained.
- Email the saved PDF as an attachment to richardfricks@gmail.com using the subject line: West Hollow Day-Use Agreement — Your Full Name
If you complete the Agreement by hand, print all five pages and email clear photographs or scans of every completed page.
Oak Hollow Cabins will review the Agreement, complete the Owner/Host signature and date fields, and return an accepted copy to you. Please keep that copy, the Guest Rules, and your reservation confirmation for your records.
Important
This Agreement is for day use only. It does not authorize an overnight stay, residential occupancy, unrestricted access to the 70-acre property, or use of any area or activity not specifically approved by Oak Hollow Cabins.
